Abstract (English): Introduction. The incidence of choledocholithiasis in pediatrics has increased in the last 20 years. Treatment typically involves two procedures --endoscopic retrograde cholangiopancreatography (ERCP) and laparoscopic cholecystectomy (LC). The objective of this study was to demonstrate that conducting both procedures in a single step is safe in pediatric patients. Materials and methods. A prospective study was carried out in two Argentinian institutions over three years. Patients under 18 years of age diagnosed with choledocholithiasis were included. They were divided into two groups --surgery (one-step ERCP+LC) and control (two-step ERCP+LC). Results. 42 patients were included. The surgery group consisted of 27 patients, whereas the control group was made up of 15 patients. Both groups were similar in terms of age and sex. The results showed that median anesthetic time was significantly shorter in the surgery group (120 minutes) than in the control group (150 minutes). 2 patients from the control group developed cholecystitis while awaiting LC. Conclusions. Conducting ERCP+LC in a single step is safe in pediatric patients and can reduce anesthetic times as well as cholelithiasis morbidity and complications. [ABSTRACT FROM AUTHOR]
Abstract (Spanish): Introducción. La incidencia de la litiasis coledociana en pediatría ha aumentado en los últimos 20 años. El tratamiento habitual de la misma implica dos procedimientos: colangiopancreatografía retrógrada endoscópica (CPRE) y colecistectomía laparoscópica (CL). Este estudio busca demostrar que realizar ambos procedimientos en un solo tiempo es seguro en pacientes pediátricos. Material y métodos. Se realizó un estudio prospectivo en dos centros de Argentina durante tres años, incluyendo pacientes menores de 18 años con diagnóstico de litiasis coledociana. Se dividieron en dos grupos: intervención (CPRE y CL en un tiempo) y control (CPRE y CL en dos tiempos). Resultados. Se incluyeron 42 pacientes; 27 conformaron el grupo "intervención" y 15 el grupo "control". Ambos grupos fueron similares en cuanto a edad y sexo. Los resultados mostraron que la mediana de tiempo de anestesia fue significativamente menor en el grupo intervención (120 minutos) en comparación con el grupo control (150 minutos). Dos pacientes del grupo control presentaron colecistitis mientras esperaban el turno quirúrgico para la CL. Conclusiones. Realizar CPRE y CL en un solo tiempo es seguro en pacientes pediátricos y puede reducir el tiempo de anestesia y la morbilidad asociada a la litiasis vesicular y sus complicaciones.
