Over the past decades, significant progress has been made on our understanding of the possible mechanisms involved in hantavirus transmission in the southern hemisphere and how to address and treat hantavirus infections. Hantavirus infections have been subject to controversies concerning its prevention, case manage ment and treatment. Some possibly related to the fact that it is a rare disease and there is lack of comprehen sive data regarding treatment options. Besides, the pos sible person-to-person transmission of some genotypes (only proven in certain regions of Argentina and Chile) has proven to be a challenge for infection control. In addition, the dynamic scenario regarding emerging infectious diseases in the region has produced changes in the possible differential diagnoses to be considered. This review intends to summarize the current available scientific evidence on some of the remaining controver sies regarding hantavirus infections in Argentina and South America. It is a narrative literature of published evidence reviewed by a group of experts with the unique experience of having faced outbreaks in this region with interhuman transmission. It discusses respiratory precautions adapted to different genotypes/areas, pos sible transmission through breastfeeding, predictors of severity in laboratory tests, the use of corticosteroids, plasma and ribavirin for treatment, and finally differen tial diagnoses in Argentina. While most of these aspects of hantavirus infection are still complex to conclude, it is essential to raise awareness in these severe infections of the available scientific evidence in order to prevent and improve the quality of care, optimizing patient outcomes. En los últimos años se han producido avances para una mejor comprensión de los posibles mecanismos de transmisión, abordaje y terapéutica en las infecciones por hantavirus. Sin embargo, debido a que es una en fermedad infrecuente y a ciertas particularidades en la transmisión de algunos genotipos, como la transmisión persona a persona (únicamente demostrada en ciertas regiones de Argentina y Chile), aún persisten contro versias respecto a su prevención, manejo y tratamiento. Además, el escenario dinámico respecto a las enferme dades infecciosas emergentes en la región ha producido cambios en los posibles diagnósticos diferenciales a considerar. Este documento se propone actualizar y abordar, desde la revisión bibliográfica narrativa de la evidencia publicada por un grupo de expertos, con la experiencia particular de haber enfrentado brotes con transmisión interhumana, algunas de las controversias que aún despiertan las infecciones por hantavirus en la Argentina y en América del Sur. Se discuten las re comendaciones de aislamiento adaptadas a diferentes genotipos, mecanismos de transmisión, y área geográ fica, la posible transmisión a través de la lactancia, los predictores de gravedad en el análisis de laboratorio, el uso de corticoides, plasma y ribavirina para el trata miento, y finalmente los diagnósticos diferenciales en Argentina. Si bien en la mayoría de estos aspectos de la infección por hantavirus aún resulta complejo arribar a una conclusión, es esencial conocer la evidencia disponi ble, a fin de adoptar las mejores conductas posibles en la prevención, diagnóstico y tratamiento de las infecciones por hantavirus.
