This article evaluates the clinical effectiveness of universal antenatal screening and treatment for toxoplasmosis in a referral hospital in Northern Italy over a ten-year period (2011–2021). Among 347 pregnant women referred for suspected toxoplasmosis, 15 had confirmed infection and 141 had suspected infection; all confirmed cases and most suspected cases received antibiotic treatment. Amniocentesis was performed in 118 cases with no positive results, and vertical transmission occurred in only one confirmed case (7% incidence), with no cases among suspected infections. The study highlights the efficiency of current screening and management protocols but raises concerns about potential overuse of invasive procedures and pregnancy terminations in low-risk cases, suggesting the need for refined counseling and management strategies in light of changing epidemiology.
