Abstract (English): Aim: Carbon monoxide (CO) poisoning is a significant cause of morbidity and mortality in childhood. CO has been called the 'silent killer' due to its colorless, tasteless and non-irritating properties. The aim of this study was to evaluate the demographic, clinical and laboratory characteristics of patients presenting to the pediatric emergency department with CO poisoning. Methods: This study is a retrospective study conducted in a tertiary pediatric emergency department. Patients presenting to the pediatric emergency department with a diagnosis of CO poisoning between July 2017 and July 2022 were evaluated. CO poisoning cases were classified as mild with COHb (carboxyhemoglobin) levels of 10-20%, moderate with levels between 20-50%, and severe with levels above 50%. Patients were divided into two groups according to their COHb levels at the time of initial presentation: mild CO poisoning (Group I) and moderate/severe CO poisoning (Group II). The control group was selected from healthy children group in the study. Results: 164 children (82 patients and 82 controls) were included in the study. The median age of CO poisoning patients was 108 (IQR; interquartile range: 48-153) months and the control group was 116 (IQR71-160) months (p=0.529). The median COHb level of the patients was 18.9 percent (1-60). According to COHb levels, mild poisoning was n=42 (51.2%) while moderate and severe poisoning was n=40 (48.8%). The most common symptoms due to carbon monoxide poisoning were nausea (n=32, 39%), vomiting (n=29, 35.3%), altered consciousness (n=20, 24.3%), syncope (n=17, 20.7%) and headache (n=12, 14.6%). When the hemogram parameters of 82 patients with carbon monoxide poisoning and the control group were compared, it was determined that WBC (White blood cell), ANC (Absolute neutrophil count), NLO (Neutrophil-to-lymphocyte ratio) and SII (Systemic immune-inflammation index) values were higher and MPV (mean platelet volüme) values were lower in patients with CO poisoning. Conclusion: As a result, the most common symptoms in children presenting with CO poisoning were nausea and vomiting. In patients diagnosed with CO poisoning, WBC, ANC, NLO and SII levels were significantly higher than the control group. [ABSTRACT FROM AUTHOR]
Abstract (Turkish): Amaç: Karbon monoksit (CO) zehirlenmesi çocukluk çağında önemli bir morbidite ve mortalite nedenidir. Renksiz, kokusuz, tatsız ve tahriş etmeyen özellikleri nedeni ile CO, 'sessiz katil' olarak adlandırılmıştır. Bu çalışmanın amacı, çocuk acil servisine CO zehirlenmesi ile başvuran hastaların demografik, klinik ve laboratuvar özelliklerinin değerlendirilmesidir. Yöntemler: Bu araştırma, üçüncü basamak bir çocuk acil servisinde yapılan retrospektif bir çalışmadır. Temmuz 2017 ile Temmuz 2022 tarihleri arasında, çocuk acil kliniğine CO zehirlenmesi ile başvuran hastalar değerlendirildi. CO zehirlenmesi olguları COHb (karboksihemoglobin) düzeyi %10-20 olan hafif, %20-50 arasında olanlar orta, %50 üzerinde olanlar ise ağır zehirlenme olarak sınıflandırıldı. Hastalar ilk başvuru anındaki COHb düzeylerine göre, hafif CO zehirlenmesi (Grup I) ve orta/ağır CO zehirlenmesi (Grup II) olarak iki gruba ayrıldı. Kontrol grubu çalışmadaki hasta grubu ile benzer yaş ve cinsiyetteki sağlıklı çocuklar arasından seçildi. CO zehirlenmesi grupları laboratuvar ve klinik bulgular açısından karşılaştırılırken, CO zehirlenmesi olguları ve kontrol grubu laboratuvar bulguları açısından karşılaştırıldı. Bulgular: Çalışmaya 164 çocuk (82 hasta ve 82 kontrol grubu) dâhil edildi. CO zehirlenmesi olan hastaların ortanca yaşı 108 ay (ÇAD; çeyrekler arası dağılım; 48-153), kontrol grubunun ise 116 ay (ÇAD; 71-160) idi (p=0.529). Hastaların ortanca COHb düzeyi yüzde 18,9 (1-60) idi. COHb düzeylerine göre hastaların 42'si (%51,2) hafif zehirlenme iken; 40'ı (%48,8) orta veya şiddetli zehirlenme grubundaydı. Karbon monoksit zehirlenmesine bağlı en sık görülen semptomlar bulantı (n=32, %39), kusma (n=29, %35,3), bilinç değişikliği (n=20 %24,3), senkop (n=17, %20.7) ve baş ağrısı (n=12, %14,6) idi. Karbon monoksit zehirlenmesi olan 82 hastanın hemogram parametreleri, kontrol grubu ile karşılaştırıldığında, WBC (beyaz küre sayısı), ANC (mutlak nötrofil sayısı), NLO (nötrofil lenfosit oranı) ve SII (sistemik immün inflamasyon indeksi) değerlerinin daha yüksek, MPV (ortalama trombosit hacmi) değerlerinin ise daha düşük olduğu saptandı. Sonuç: Sonuç olarak, CO zehirlenmesi ile başvuran çocuklarda en sık görülen semptomlar bulantı ve kusmaydı. CO zehirlenmesi tanısı konulan hastalarda WBC, ANC, NLO ve SII düzeyleri kontrol grubuna göre anlamlı yüksekti. [ABSTRACT FROM AUTHOR]
