Abstract (English): Palliative care aims to improve the quality of life of patients with advanced serious illnesses and their relatives. In Mali, its integration into surgical services remains inadequate despite the rising incidence of late-diagnosed cancers. Objective: To assess palliative care in the general surgery department at CHU Gabriel Touré. Methods: Cross-sectional descriptive study (March-August 2025) including 102 participants (30 patients, 30 caregivers, 42 healthcare providers). Data were collected using questionnaires and analyzed with Excel, SPSS, and Epi Info. Results: Palliative care accounted for 8.7% of hospitalizations. Mean patient age was 55.4 years (female predominance). Advanced digestive cancers (gastric, pancreatic) were predominant. Pain was reported in 93.3% of patients, but use of strong opioids (morphine) was limited. Major needs included accompaniment, pain relief, and psychological/spiritual support. Healthcare providers had adequate theoretical knowledge but lacked practical training, standardized protocols, access to essential medications, and dedicated units. Conclusion: Urgent need to structure palliative care through dedicated units, adapted protocols, and enhanced staff training to optimize quality of life. [ABSTRACT FROM AUTHOR]
Abstract (French): Les soins palliatifs visent à améliorer la qualité de vie des patients en phase avancée de maladies graves et de leurs proches. Au Mali, leur intégration en chirurgie reste insuffisante, malgré l'augmentation des cancers diagnostiqués tardivement. Objectif : Évaluer les soins palliatifs au service de chirurgie générale du CHU Gabriel Touré. Méthode : Étude transversale descriptive (mars-août 2025), incluant 102 participants (30 patients, 30 accompagnants, 42 soignants). Données collectées par questionnaires et analysées via Excel, SPSS, Epi Info. Résultats : Ces soins concernaient 8,7 % des hospitalisations. Âge moyen des patients : 55,4 ans (prédominance féminine). Cancers digestifs avancés (gastriques, pancréatiques) prédominants. Douleur chez 93,3 % des patients, mais recours limité aux opioïdes forts (morphine). Besoins majeurs : accompagnement, analgésie, soutien psychologique/spirituel. Connaissances théoriques des soignants correctes, mais lacunes en formation pratique, protocoles, accès médicamenteux et unités dédiées. Conclusion : Nécessité urgente de structurer ces soins via unités dédiées, protocoles et formations renforcées pour optimiser la qualité de vie. [ABSTRACT FROM AUTHOR]
