Tularemia is a rare zoonotic infection that may clinically and radiologically mimic pulmonary malignancy and represent a diagnostic pitfall. We report the case of a 54-year-old man with a fever and a markedly 18-fluorodeoxyglucose-avid pulmonary lesion with mediastinal lymphadenopathy on PET/CT, raising suspicion for lung cancer. Bronchoscopic examinations, including endobronchial ultrasound -guided fine-needle aspiration and radial endobronchial ultrasound, revealed non-caseating granulomatous inflammation without evidence of malignancy. The diagnosis was confirmed serologically. The disease had a self-limited course with spontaneous resolution. This case highlights the importance of considering infectious causes in PET-positive pulmonary lesions and the role of targeted serology to avoid unnecessary invasive procedures. Tularémie je vzácná antropozoonóza, která může klinicky i radiologicky imitovat maligní plicní proces a představovat diagnostickou past. Předkládáme kazuistiku 54letého muže s febriliemi a výrazně 18-fluorodeoxyglukóza avidním plicním ložiskem s mediastinální lymfadenopatií na PET/CT, suspektním z primárního plicního karcinomu. Bronchoskopická vyšetření včetně endobronchiálního ultrazvuku s aspirační biopsií tenkou jehlou a radiálního endobronchiálního ultrazvuku prokázala nekaseifikující granulomatózní zánět bez známek malignity. Diagnóza byla potvrzena sérologicky. Onemocnění mělo samolimitující průběh se spontánní regresí nálezu. Kazuistika zdůrazňuje nutnost zvažovat infekční etiologii u PET pozitivních plicních infiltrátů a význam cílené sérologie k prevenci nadbytečných invazivních výkonů. [ABSTRACT FROM AUTHOR] |
