Description: This report documents the initial phase of Project HEAR (Health Enrollment Assessment Review). The initial phase of the project examined the feasibility of developing an enrollment questionnaire for use in the Region VI TRICARE Prime, a health maintenance organization (HMO) which will become operational in 1995. The questionnaire will have three functions: assess preventive service needs of enrollees; predict which enrollees potentially will utilize high levels of medical resources or primary care manager (POM) time; and recommend the appropriate level of POM. Each function for Project HEAR is discussed separately in this report: (1) assessment of preventive service needs of enrollees, (2) prediction of high resources or PCM time utilization, and (3) assignment of appropriate level of POM.
