Description: Background: In 2019, Reunion Island experienced an outbreak of dengue causing a high hospital burden. Diabetes, highly prevalent in Reunion Island, is associated with a risk of severe dengue. In this study, we sought to find criteria at hospital admission that will predict severe dengue in diabetic patients.Method: We retrospectively analyzed demographic, clinical, and biological parameters in the cohort of patients consulting the hospital for dengue between January and June 2019. Patients with diabetes were compared in bivariate and multivariate analyses. Results: Of the 936 patients included, 188 (20%) developed severe dengue. Among the 184 diabetic patients, 60 (33%) developed severe dengue and 128 (17%) among the non-diabetic patients. The diabetic subjects were older and had more associated comorbidities compared with the non-diabetic patients. In the multivariate analysis, only a history of chronic kidney disease was associated with severe dengue with an OR of 2.26 (95%CI: 1.03-4.99, p=0.042) and no significant difference in terms of clinical and biological signs at admission was found between severe and non-severe dengue in diabetic patients in our study. Conclusion: A history of chronic kidney disease in diabetic subjects is predictive of severe dengue, there is no significant difference at admission in terms of clinical and biological signs between diabetic subjects who had severe and non-severe dengue. Future prospective studies with a larger sample size should be considered to confirm these results.
