INTRODUCTION AND AIMS: FGF-23 is involved in the regulation of serum phosphate levels. So far no studies have been published which investigated the effect of rapid changes of serum phosphate concentrations during dialysis on FGF-23 levels. The aim of this study was to evaluate FGF-23 serum levels during haemodiafiltration (HDF) and its relationship to other parameters. METHODS: Fourteen male dialysis patients were included into the study. Patients underwent two different pre-dilution online haemodiafiltration (OLHDF) sessions (5008 online plus machine, FMC) with either 1.25 mmol/L or 1.0 mmol/L dialysate Ca (DCa). Blood flow was ~ 300 mL/min, dialysate flow rate was 500 mL/min and substitution flow was adjusted automatically. The duration of each session was four to six hours according to individual prescription. Blood samples were taken at short intervals, only serum concentrations from the start and the end of the session are presented in this abstract. Spearman correlation coefficients (rho) were calculated to investigate linear relations between variables. RESULTS: Mean age of the patients was 49±15 years, time on dialysis 73±91 months, and mean BMI was 27.6±14.2 kg/m2. The eKt/V was 1.4±0.3 for the HDF session with 1.25 mmol/L DCa and 1.4±0.4 for the session with 1.0 mmol/L DCa (p=0.917). FGF-23 serum levels correlated to phosphate levels (rho=0.445; p=0.018), to 1,25-OH-D3 levels (rho= -0.404; p=0.033), and to 25-OHD3 levels (rho= -0.401; p=0.002). FGF-23 levels did not correlate to iPTH levels. Patients showed wide ranges in FGF-23 levels. Higher FGF levels were accompanied by higher pre dialysis PO4 levels (p=0.002). In all patients a significant decrease of FGF-23 serum levels was observed during HDF with 1.0 mmol/L DCa. Changes in PO4 levels during HDF showed a borderline significant correlation to the changes in FGF-23 levels (rho= -0.369; p=0.053). Changes in phosphate and FGF-23 during OLHDF |
