In foreign countries, mobile x-ray already is being used where transfer to the radiology department at the hospital may be an obstacle. Using mobile x-ray, the x-ray examination is performed in the patients own home with transportable equipment. Only few patients needed repeated radiography and radiologists were able to diagnose the patients using images performed with portable x-ray equipment. A cluster-randomized study found that portable x-ray could reduce hospitalizations and health care costs for nursing home residents with pneumonia because fewer patients were hospitalized. There were several limitations of the studies but the most important in the decision making of proceeding mobile x-ray was that none was randomized except for a pilot study, none measured patient’s reactions and none systematic measured economic consequences of implementing mobile x-ray including a broad pathology. At The Department of Radiology (DOR), Aarhus University Hospital there is an ongoing pilot project mobile x-ray for patients living in nursing homes in Aarhus. A radiograph examines the patient in the patients own home and radiologists at DOR evaluate the images. The pilot project has been going on for almost 2 years without problems providing the clinical quality of the images is acceptable. After one year, mobile x-ray was evaluated inspired by the framework of Triple Aim and concluded as other studies that mobile x-ray could be used outside the hospital. The Institute for Health Care Improvement has developed the framework Triple Aim that describes an approach to optimize health care performance for a specific population. It contributes to improve the effect of an intervention and has three dimensions; improved health of the defined population, experience of care and reduce or at least control the per capita costs. |
