The aim of this study is to assess perioperative infectious risk factors in patients treated with surgery for Benign Prostatic Hyperplasia (BPH). We conducted an observational, multicenter and retrospective study including 380 patients treated with surgery for prostatic desobstruction from January 2016 to April 2021. The primary endpoint was the occurrence of an infectious complication occurring within thirty days after surgery. Most patients were treated with endoscopic surgery (mono or bipolar TURP, prostatic photovaporization, prostatic enucleation with Holmium or Greenlight laser), 5.5% with open adenomectomy. The rate of postoperative infections was 4.5%, all infected patients had a preoperative positive urine culture, but only 76.5% received preoperative antibiotic therapy. Risk factors for postoperative infection are: urinary tract infection the year before the surgery, urinary catheter, significant leukocyturia, and a preoperative positive urine culture. Nevertheless, the duration of the surgery, the age of the patient and the absence of antibiotic prophylaxis are not predisposing factors for a urinary tract infection. There is no significant difference between the different surgical techniques for postoperative infection. This study confirms that postoperative urinary tract infections are independent of the BPH surgical technique but are more related to prophylactic measures and the characteristics of preoperative urine culture which are difficult to apply. |
