International audience ; The inappropriate use of proton pump inhibitors (PPIs) can be associated with serious adverse effects and have an impact on healthcare costs. Deprescribing should be considered when inappropriate PPIs prescription is identified. Deprescribing interventions are seldom targeted towards patients. Those solely aimed at prescribers have limited efficacy.Objective. The objective of this trial is to evaluate the efficacy to deprescribe PPIs of a multifaceted intervention conducted towards patients and their general practitioners.Methods. This is a pragmatic, cluster-randomized, controlled trial conducted in the French Western region Pays de La Loire. General practitioners and their adult patients who were prescribed more than 300 defined daily doses (DDD) of PPIs in the year prior the start of the study will be included. In total, 2,500 general practitioners and 64,500 patients will be cluster-randomized by general practitioners practices. Three interventions will be compared: i) a multifaceted intervention combining a) an education brochure on PPIs deprescribing mailed to patients and b) a visit from a French health insurance representative (DAM) to the general practitioners regarding PPI deprescription, ii) an intervention with solely a DAM visit, and iii) no intervention.Expected results. The primary outcome will be PPI deprescribing, defined as the proportion of patients who achieve at least a 50% reduction in the quantity of PPIs prescribed (DDD/year) at 12 months compared to before inclusion. Based on previous trials, we anticipate a 10% increase in PPI deprescribing with the multifaceted intervention compared to the intervention targeting solely general practitioners and the control group. The study was funded through the Resp-IR 2021 call for proposals. It was launched in October 2022, and preliminary results are expected in 2024. ; Introduction.
