Trigeminal autonomic cephalalgia (TAC) is a group of primary headache disorders characterized by unilateral moderate to severe pain in the periorbital and temporal region with associated ipsilateral cranial autonomic features. It is important to differentiate these headaches from potentially dangerous secondary headache mimics, including headaches with vascular, neoplastic, ocular, and pituitary-related causes. TAC headaches are differentiated by the duration and frequency of attacks, subtle differences in associated features, and response to specific treatments. The pathophysiology of TACs is thought to involve the trigeminovascular afferent system as well as the hypothalamus, which is thought to be the source of the diurnal variation of some of the TAC subtypes as well as the cranial autonomic features. |
