Description: Background: Rheumatoid arthritis (RA) is a chronic, systemic, inflammatory disease, which is associated with increased cardiovascular mortality. Aortic stiffness is an independent marker and prognosticator of cardiovascular risk. Aim of the study: We investigated whether there is an effect of treatment with the TNF antagonist adalimumab on aortic stiffness in RA patients and whether that effect is independent of the response to treatment. Furthermore, we examined whether there is a differential effect on aortic stiffness among patients treated with a classic disease modifying anti-rheumatic drug (DMARD) i.e. methotrexate. Methods: A total of 130 patients were included in the study but finally we studied 36 RA patients (age 60.08 ± 11.92 yrs) with active disease (disease duration 6.22 ± 6.16 yrs), who received either adalimumab (± DMARDs, n = 18) or methotrexate (n=18) for 12 weeks. All patients underwent evaluation of clinical status and inflammatory markers such as CRP and ESR, at baseline and at the end of treatment period. DAS 28 score was used an inidicator of RA disease activity. Carotid-femoral Pulse Wave Velocity (PWV) was measured as an index of aortic stiffness. Τhe group that received adalimumab was further divided into two subgroups according to the response to treatment (DAS28 decrease > or < 1.2). The changes in measures of disease activity, aortic stiffness and inflammatory state from baseline to 12 weeks were compared between the groups and subgroups.Results: PWV decreased in patients that received adalimumab over 12 weeks (from 8.2±2 to 7.0±1.8 m/s, P
