Description: Physiotherapy has been shifting shared decision making (SDM) from rhetoric to the reality of business as usual in NHS Wales, supported by a ‘train the trainer’ education programme and quality improvement initiative. The programme has spread through musculoskeletal physiotherapy services in five health boards and one trust and is gaining further momentum and sustainability through a national e-learning platform. Through the lens of the National Institute for Health and Care Excellence clinical guideline on SDM, this chapter reflects on the authors’ experience. It shares successes in training and quality improvement and describes the importance of policy/organizational support, clinical leadership, communication skills, and clinical reflection. It reflects on the need for clinical teams to communicate uncertainty and complexity using SDM, to improve quality of care for unique individuals and workforce sustainability. It describes concerns about the current direction of research, including its ability to provide the data required to create and utilize decision aids in an increasingly complex environment, and makes recommendations.
