EVIDENCE REVIEW Ataskforceofexpertsinintensivecaremedicineandepidemiologygenerated definitionsoforgandysfunction,identifiedrelevantvariables(physiologicalandlaboratorydata specifictotheorgansystem,pharmacologicalandmechanicalorgansupport),andproposeda0to 4–pointgradingofdysfunctionseveritythroughmeetings,Delphiprocesses,andexplicitrules, informedbydatasynthesis,includingsystematicreviewsandmeta-analysis.Variablesweretestedin 2validationexercisesusingseparatedatasetstotaling3.34millionpatientswithin10representative databasesfromdiversegeographicalandsocioeconomicsettingstoassessdistributionand predictivevalidity(mortalityatintensivecareunitdischarge).
